A tibial plateau fracture is a break in the upper portion of the tibia (shin bone) where it forms part of the knee joint. It is most often caused by high-impact trauma such as motor vehicle accidents or sports injuries in younger adults, and by falls in older adults — particularly those with osteoporosis. Common symptoms include knee pain, swelling, bruising, and difficulty bearing weight on the affected leg. Diagnosis is confirmed with X-ray, CT, or MRI imaging. Treatment ranges from rest, splinting, and non-weight-bearing for minor fractures to surgical repair with plates and screws for displaced or severe breaks. Most patients return to normal activity within 3 to 6 months, depending on injury severity and treatment approach.

In younger adults, most tibial plateau fractures are a result of high-velocity trauma to the leg, such as automobile accidents or high impact sports. In the elderly, tibial fractures occur more commonly with falls. Risk factors, especially in the elderly, for tibial plateau fractures include infection and osteoporosis (thinning of the bone).

Tibial Plateau Fracture Symptoms

The most common symptoms of tibial plateau fracture is pain or discomfort near the upper part of your shin following an impact injury or fall.  Other symptoms can include:

  • difficulty bearing weight on your leg
  • bruising or swelling
  • paleness of the lower leg as a result of decreased blood flow
  • pain in the joint with or without weight bearing

Your doctor can provide a proper diagnosis by conducting imaging tests such as an X-ray  or possibly MRI or CT scan.

Treatment

Treatment for tibial plateau fractures depends on the severity and type of break in the bone. For more minor injuries, non-surgical treatment options include:

  • resting the joint
  • splinting the knee and avoiding weight-bearing
  • taking pain medications
  • applying ice and elevating the extremity while the bone heals

If surgery is necessary, your surgeon will likely use screws and plates to properly align the bone. Surgical correction of tibial plateau fractures typically is able to restore function in the injured extremity, though the recovery process can be prolonged.

Recovery time varies for tibial plateau fractures depending on the severity of the injury and treatment method required, but return to normal activity is typically possible within 3-6 months.

How long does it take to recover from a tibial plateau fracture?
Recovery time for a tibial plateau fracture varies based on fracture severity and treatment approach. Minor fractures treated non-surgically typically require 8-12 weeks for initial healing, while complex fractures requiring surgical repair may take 3-6 months for full recovery. During this time, you’ll progress through phases including non-weight-bearing rest, gradual weight-bearing with assistive devices, and physical therapy to restore strength and range of motion. Athletes or individuals with physically demanding jobs may need additional rehabilitation time before returning to full activity. Your orthopedic surgeon will monitor healing through follow-up X-rays and adjust your recovery timeline based on bone healing progress.
Walking on a tibial plateau fracture is generally not recommended without medical clearance, as weight-bearing can worsen the injury and lead to complications. Immediately after injury, attempting to walk typically causes severe pain and further displacement of bone fragments. For non-displaced or minor fractures, your doctor may allow partial weight-bearing with crutches or a walker after initial healing. However, complex or displaced fractures usually require a period of complete non-weight-bearing for 6-12 weeks to allow proper bone healing. Long-term complications from tibial plateau fractures can include post-traumatic arthritis, chronic knee pain, reduced range of motion, and joint instability. Post-traumatic arthritis is the most common complication, developing when cartilage damage from the initial injury causes progressive joint degeneration over time. Some patients experience persistent stiffness or inability to fully straighten or bend the knee, particularly after complex fractures requiring prolonged immobilization. Malunion (improper bone healing) or nonunion (failure to heal) can occur if the fracture isn’t adequately stabilized. Nerve or blood vessel damage, though rare, can cause numbness or circulation problems. Early surgical intervention for displaced fractures and adherence to physical therapy protocols significantly reduce complication risks.
Doctors classify tibial plateau fractures using the Schatzker classification system, which divides fractures into six types based on fracture pattern and location. Type I involves a simple split fracture of the lateral (outer) plateau, common in younger patients. Type II combines a split fracture with depression of the joint surface. Type III is a pure depression fracture without splitting. Type IV affects the medial (inner) plateau, often associated with more severe trauma. Type V is a bicondylar fracture affecting both sides of the plateau. Type VI includes fractures extending below the plateau into the shaft. This classification helps orthopedic surgeons determine appropriate treatment—simpler Type I or III fractures may heal with non-surgical care, while complex Type V or VI fractures typically require surgical reconstruction with plates and screws.
