What causes arthritis joint pain? In my practice at the Bone and Joint Institute of Tennessee, that is one of the first questions patients ask me. The short answer: arthritis joint pain starts when something damages the joint itself, and five culprits cause most of that damage.
After more than twenty years as a hip and knee replacement surgeon, I have sat across the desk from thousands of patients with aching joints. Here is what I tell them about why joints hurt, what makes the pain flare, and what actually helps.
What Causes Arthritis Joint Pain?
Arthritis joint pain comes from five main causes: cartilage wear, an overactive immune system, uric acid crystals, old joint injuries, and infection. Each one damages the joint in a different way, and each one feels a little different to the patient living with it.
Arthritis is not one disease. It is an umbrella term for more than 100 conditions that affect the joints. A joint is simply the place where two bones meet, cushioned by cartilage and bathed in lubricating fluid.
- Cartilage wear (osteoarthritis): the protective cushion on the ends of your bones thins out over decades of use.
- Immune system attack (rheumatoid and psoriatic arthritis): your body’s own defenses mistakenly inflame the joint lining.
- Uric acid crystals (gout): excess uric acid in the blood forms sharp crystals inside the joint.
- Old injuries (post-traumatic arthritis): a ligament tear or fracture from years ago sets the stage for arthritis later.
- Infection (septic arthritis): bacteria reach the joint and cause sudden, intense swelling and pain.
What Is Osteoarthritis and Why Is It the Most Common Cause?
Osteoarthritis is wear-and-tear arthritis, and it is the most common cause of joint pain by a wide margin. Studies suggest roughly half of all adults will develop osteoarthritis in at least one joint during their lifetime.
I think of it the way I think of tire tread. Decades of walking, climbing, kneeling, and lifting slowly wear down the cushion inside the joint. Some wear is a normal part of aging; pain begins when the wear outpaces the joint’s ability to keep up.
How does cartilage loss cause arthritis joint pain?
Cartilage loss causes arthritis joint pain by removing the smooth, slippery layer that lets bones glide past each other. As that layer thins and frays, the underlying bone takes more of the load. The joint becomes stiff, achy with activity, and often worst at the end of the day.
In advanced cases, bone rubs directly on bone. That is the deep, grinding pain that brings most of my patients in for a hip or knee consultation.
Which joints does osteoarthritis affect most?
Osteoarthritis most often affects the weight-bearing joints: the knees and hips. It also shows up in the hands, shoulders, feet, and spine. My surgical practice focuses exclusively on the hip and knee. For arthritis in the shoulder, foot and ankle, or spine, my fellowship-trained colleagues at the Bone and Joint Institute of Tennessee provide that specialized care under the same roof.

How Does Inflammatory Arthritis Cause Joint Pain?
Inflammatory arthritis causes joint pain when the immune system attacks the joint lining by mistake. Rheumatoid arthritis is the best-known example. The inflammation typically strikes matching joints on both sides of the body, and the pain and stiffness are often worst in the morning or after rest.
Rheumatoid arthritis affects more than 1.3 million Americans, and about 75 percent of them are women. When I see a pattern that suggests inflammatory disease, I involve a rheumatologist, a physician who specializes in immune-driven joint conditions, because early treatment can protect the joint from permanent damage.
What about gout?
Gout is inflammatory arthritis with a chemical cause rather than an immune one. When uric acid builds up in the blood, it can form needle-like crystals inside a joint. The classic story: a patient goes to bed feeling fine and wakes up with a big toe or knee so painful they cannot bear a bedsheet touching it.
What Triggers Arthritis Flare Ups?
The most common arthritis flare up triggers are over-activity, weight gain, stress, poor sleep, certain foods, and weather changes. A flare up is a stretch of days when joint pain, stiffness, or swelling suddenly gets worse, and it usually settles within a few days to a week.
In my experience, flares rarely come out of nowhere. A long weekend of yard work, a stressful month, or a string of short nights often shows up in the joints before patients connect the dots.
Does weather make arthritis joint pain worse?
Yes, weather can make arthritis joint pain worse for many people. Falling barometric pressure, cold, and damp conditions are the usual offenders. Here in Middle Tennessee, plenty of my patients tell me their knees announce a storm before the forecast does. The science on why is still unsettled, but the pattern is real enough that I take it seriously.
What Raises Your Risk of Arthritis Joint Pain?
The biggest risk factors for arthritis joint pain are age, excess body weight, previous joint injuries, family history, smoking, and jobs or sports with repetitive joint stress. You cannot change your age or your genes, but several of these are within your control.
Weight deserves special mention. Every extra pound multiplies the force crossing your knees with each step, so even modest weight loss takes meaningful pressure off painful joints. Quitting smoking and protecting your joints during high-impact work or sport also lower your long-term risk.
When Should You See a Doctor About Joint Pain?
See a doctor if joint pain lasts more than a few days, starts without a clear reason, comes with swelling, redness, or warmth, or limits your daily activities. One situation is urgent: a hot, swollen joint with fever can signal infection and needs same-day medical attention.
Patients come to see me from Franklin, Nashville, Columbia, and communities across Middle Tennessee, often after months of wondering whether their pain is “bad enough” to mention. My advice is simple: if a joint is changing how you live, it is worth an evaluation. An exam and an X-ray usually tell us exactly what we are dealing with.
What Helps Arthritis Joint Pain?
Movement, weight management, physical therapy, anti-inflammatory medication, and joint injections relieve most arthritis joint pain without surgery. There is no cure for arthritis, but it can almost always be managed, and staying active is the single most protective habit I recommend.
Walking, cycling, swimming, and strength training keep the muscles around a joint strong, which takes load off the damaged surfaces. When patients need more help, I start with nonsurgical joint pain treatment options and reserve surgery for joints that no longer respond.
When severe hip or knee arthritis reaches that point, total hip replacement and total knee replacement are two of the most successful operations in modern medicine. In my practice, I perform more than 700 joint replacements each year, many with robotic-assisted guidance for precise implant placement.
